Intake is the program dedicated to educating families about the various supports and services available to eligible individuals. It also helps families understand the terms and acronyms used within the service delivery system. These services include:
Respite: Provides the primary caregiver with personal time while ensuring that a qualified caregiver looks after the individual.
Community Habilitation (Community-Hab): Aims to assist individuals with Activities of Daily Living (ADL) and enhance their independent living skills.
Afterschool Programs: Designed to give individuals with disabilities opportunities to socialize with peers, participate in recreational activities, and receive additional educational support.
The Intake Coordinator will assist families in accessing services for their loved ones with developmental disabilities until a Medicaid Service Coordinator is appointed or assigned.
The Intake Coordinator’s responsibilities include helping families schedule and obtain psychological and psychosocial evaluations, updated medical records, and necessary documents such as birth certificates, Social Security cards, Medicaid cards (if applicable), and proof of address.
Additionally, the Intake Coordinator will prepare the Level of Care (LOC) package, which includes psychological and psychosocial evaluations, updated medical records, and a transmittal form to be submitted to the local DDSO for OPWDD approval.
